PAIN WITH OBVIOUS CAUSES: A SCRATCH
A scratch, by definition, is a trivial injury, but it contains many of the pain-producing components found in much more serious conditions. At the instant of the scratch, pressure has opened channels in the nerve fibres in the skin and generates nerve impulses that begin their travel to the spinal cord. Pressure, cold, heat and chemicals are the four ways in which sensory nerve impulses can be initiated. The scratch produces an immediate stinging sensation due to pressure but this is only the first stage of a process that now proceeds. Inspection of the scratch at the time reveals a white line. Within seconds, the line becomes red and the redness spreads slowly in a flare. This is not bleeding into the skin but an opening up of the small vessels in the damaged skin and the nearby area. Even later, a bump appears in the skin along the line of the scratch. This is the wheal and is caused by clear fluid and cells moving from the blood vessels into the damaged tissue. Here, on a tiny scale, are the four signs of inflammation: redness, heat, swelling and pain.
The pain changes from the initial sting to an ache and the area, if gently pressed, is tender. Tenderness arises from two causes. Chemicals leaking from the damaged tissue have sensitized nerve fibres. This means that chemical stimulation has been added to the temporary initial pressure stimulus and prolongs the pain. The second cause of the tenderness is that cells in the spinal cord have been woken up by the initial volley of nerve impulses and are on guard. Two stages may follow the scratch with its initial phases of flare, wheal, pain and tenderness. If bacteria have been introduced, local infection grossly exaggerates the four signs of inflammation: the colour becomes angry, the swelling weeps clear fluid, which may become milky with white cells, and the pain grows. If infection does not occur, the repair process eats up the cell debris and cells migrate in to stitch the edges of the skin together to produce the white line as the mark of an old scratch.
